Children are expected to be picked up immediately following Sunday Service in the Sanctuary at Unity of Naples, and may only be released to adults authorized by parents or legal guardians whose identity has been verified by photo identification. Names, addresses, and telephone numbers of persons authorized to pick up your child are being obtained during this enrollment process and will be regularly reviewed. Please keep Unity of Naples updated on any changes to this information. Thank you for helping us keep our children safe and happy!

By clicking "Submit" below, you hereby consent to having your child(ren) interviewed, photographed, recorded on audio tape or video tape, ONLY by a staff member/volunteer of Unity of Naples for the reporting of programs and events taking place on our church campus, with full knowledge that the end product may appear in print publications, in promotional videos for the church or on our web site. The end product may also be used for instructional purposes and/or public information. You understand that your child(ren) may be depicted and or identified in one or more of these media releases. You release Unity of Naples and any member of the staff from any responsibility or recourse for the use of interviews, videotapes, sound recordings or other images either created by your child(ren) or others.

By clicking "Submit" below, you hereby consent to allowing your child(ren) to use all of the playground equipment at Unity of Naples. Equipment of the playground includes climbing structures, slides, bikes, scooters, hula hoops, balls, etc. you recognize that injuries may occur. You fully understand that the staff members/volunteers of Unity of Naples are not physicians or medical practitioners of any kind. You hereby allow the staff members/volunteers of Unity of Naples to render first aid to your child(ren) in the event of any injury or illness. Furthermore, if deemed necessary by the staff members/volunteers at Unity of Naples, you give them your permission to call 911 to seek medical help, including transportation to any health care facility or hospital. You understand that it is the express intent of Unity of Naples to provide for the safety and protection of your child(ren), and in consideration for allowing your child(ren) to play on the playground equipment you hereby release Unity of Naples and its staff members/volunteers from all liability for any and all damages and injuries suffered by your child(ren) while playing with/on the playground equipment.

You also affirm that you now have and will continue to provide proper hospitalization, health, and accident insurance coverage which you consider adequate for your child(ren)'s protection and your own protection. This acknowledgment of risk and waiver of liability, having been read thoroughly and understood completely, is agreed to voluntarily as to its content and intent.
